Only 3 employees minimum are required for your group dental plan. At your option, part-time employees may be included.
The dental plan may be offered on a completely voluntary basis (employees pay 100%) or as business-paid, or the cost may be shared in any pre-determined manner desired. (There are no added group plan fees or charges whatsoever.)
When employees leave, they can take the dental plan with them, so there is no
"COBRA" administration.

Step 2: Employee Application (pdf - opens in a new Window) one completed form for each employee (and family) joining.
Step 3: Check payable to "Careington International."
Monthly rates: Employee- $6.95, Employee plus one- $11.95, Family (3+)- $15.95.
Step 4: Mail items from Steps 1, 2, and 3 above...
1. Group Dental Plan Agreement (from Step 1 above).
2. All completed Employee Applications (from Step 2 above).
3. Check for first month's payment (total employee cost, step 3 above).

Start Date: Your group dental plan normally begins on the 1st of the month following receipt of your completed group paperwork and 1st month's payment. However, it is possible to start immediately. Please email us for assistance.

After your group dental plan is established, you will receive a plan welcome packet. Your group is billed monthly by Careington. You may add/remove employees or family members as of the first of each month. Group Change Form (pdf)
