The Plan 501 Fee Schedule applies at participating General Dentists in the following states: Alabama, Florida, Georgia, Illinois, Kansas, Louisiana, Minnesota, Mississippi, Missouri, Nebraska, Ohio, Oklahoma, Pennsylvania, Tennessee, Texas, and Virginia. Group dental plan available in all states except Montana and Vermont.
